1) Apply for non availability certificate of birth

with the  office of birth and death registrar

 

2) on basis of non availability certificate apply to judicial magistrate for registration of your birth 

 

3) you woukd need evidence of your date of birth 

 

4) affidavit of parents that you were born on said date 

 

5)  Do you have hospital discharge reports ? 

 

6) You may need said report for issuance of birth certificate after lapse of 40 years 

 

7) you would also need school leaving certificate, voter I’d card .

After passing of so many years I wonder whether you will get a birth certificate issued in your name ir not at this stage.

However if you have any proof of date of your birth then you can apply before the court of local Magistrate where you were born for such direction under Registration of birth and death Act.

You need proof of your birth date like hospital record or affidavit of parents.

The State Registrar may only file the registration of birth by the using form VS-007 “Delayed Certificate of Birth.” The top part of the form must be completed and signed by one of the registrant's parents.

After that, you will have to Apply for birth registration at the office of birth and death registrar.

  1. As per the information mentioned in the present query, makes it clear that though you were born in India and not yet been issued your birth certificate.
  2. Let me tell you that if you are educated ( which I know to sir) then you may not be needed it specifically as your 10th and 12th certificates are your birth certificate.
  3. But, still if someone is asking for the same then you can get it easily by making an affidavit and applying before the Sub-Divisional Magistrate of your district along worthy the photocopies of all the documents.
