you need to first get a marriage certificate, which means you need to legally get your marriage registered. You will have to download a form from your state government’s website and submit it at Sub Registrar Office. You will have to submit an array of photocopies of documents like Aadhar Card, PAN card etc., whose list you can find on the government’s website. Please note that marriages in India are registered under the Hindu Marriage Act, 1955.
You need to visit a registered notary and ask for an affidavit for a name change after marriage. An affidavit is basically a stamp paper notifying your intention of changing your name to a new one along with the details of previous name, a new name and the reason for a change in name.
You need to then get proof of your name change after marriage. This means that you need to have a statement made by a third party that says that you have changed your name. Although it is the third party, you need to submit the proclamation yourself. You need to do two things for this:
One submits a newspaper advertisement in at least two popular local newspapers stating that you have changed your name in the presence of a notary.
Make sure you keep a few copies of the entire newspaper with you once it is published.
The second thing is that you need to make an announcement of the name change after marriage in the Gazette of India. For that, you need to submit an application form for a name change to the Department of Publications, which works under the Ministry of Urban Development, Government of India. Gazette of India is an official government journal published by the government.
Once you are done with all the above steps, all you have to do is make the necessary changes at all the other places, like at your bank, where you may need to change your signature as a result of your name change after marriage. You will also have to make an application for a new PAN Card, Aadhar Card, etc. A uniform change at all the places with a record of the name will help in the long run.

The next step is to publish an advertisement in two newspapers- one in the official vernacular language and the other in English.
This is done to publically declare the change in the name so that if a conflict arises later, you have an official declaration of your name change.

Notifying in the Gazette
The next step is to get your name published in the State/ national gazette.
Go to the department that looks after the Gazette publication, the Government Press, in most of the cases.Fill the “Deed changing surname Form” and fill it very carefully following all the instructions mentioned onThe name changing notification will then be published in the Gazette and you’ll receive its copies in the mentioned address in the Deed Form.
With this, the name change procedure gets officially completed.

After you have successfully changed your name, you will have to update you official official documents (e.g.-Passport, PAN Card, Driving License, and Ration Card)

For changes in address in adhaar or pan card you can change through online process.
Sir she can change her address and namen based on the marriage certificate husband ID and an affidavit for address change and name. A newspaper publication has to be done after affidavit. First adhaar card and election card need to be updated based on same address and name in Passport can be changed.
1. The marriage certificate and husband's PAN along with his adhaar are sufficient to change the address of wife on her adhaar and PAN.
2. Address on Passport can also be changed on the basis of the adhaar and PAN.
Name cannot be changed but the address may be changed on submission of the change address proof.
