Copy of O.M. No. 190016 / 1 / 87 - Estt. dated 12th March, 1987 from Department of Personnel and Training.

The Ministry of Home Affairs O.M.No. 67/274/48/Estts. dated 3rd November, 1948 as amended vide O.M. No. 87/52Estt. dated 24th March, 1952 lays down the procedure for change of name by Government employees. See you can change your name by way of affidavit then after that you need to publish same in two leading newspapers and further once publication in newspaper is done you need to send the copy of affidavit along the newspaper publication copy for publishing in gazette of India.

After gazette publication your name can be changed in the records of service.

Dear Madam,

he process is not as tedious as people say it is.

Step 1: I got my marriage certificate. Make sure the marriage certificate has some serial number and address of registrar office. For us, they even verified our thumb imprints with Aadhar card number. We provided our original wedding card, Engagement photos and 2 witnesses with my valid age proof.

Duration: 1 Month

Step 2: Applied (online) for change of name and marital status in PAN. Proofs submitted were Marriage certificate, xerox of old PAN, Wedding invite). Cost for corrections in PAN is 108 rs.

Duration: I got my PAN in 2–3 weeks. The approval also goes through a verification by Income Tax Department, but it happens fast. The only thing that takes time is their delivery via Velex courier.

Step 3: Made an Affidavit (without newspaper ads). This is optional, but is accepted for Aadhar. They hardly charge 200 rs.

Duration: You will get it on the spot. 30min max.

Step 4: Changed name in Aadhar. One needs to visit Aadhar enrollment center for this change. For people who don’t know, some of the Axis Bank branches also do Aadhar modification and you can book online appointments. Aadhar correction takes 50 rs.

Proof submitted: The new original PAN card was good enough. But since my original PAN was not yet dispatched, the Aadhar center accepted my Affidavit and changed my name. Unlike eAadhar, ePAN is not yet accepted as Original PAN within India.

Duration: For me, it surprisingly took 3 workings days only. Max it will take 90 working days. One can call to 1947 helpline number for Aadhar queries.

Step 5: Once my Aadhar and PAN were in place, changing name in Bank accounts was very easy. SBI requires a written declaration stating that you want to change your name.

Duration: 3–5 working days for most banks.

Step 6: Name Change in Passport. The proof needed here are 2 Valid Government issued ID proofs and 2 newspaper cut outs. Both are Mandatory. Name Change ad should be given in 1 local and 1 National Newspaper. I used www.booknamechangead.com and release My Ad.com : The Smart Media Buying Agency to publish in TOI, The Hindu and Vijay Karnataka. Both are genuine vendors. I opted for both since I had a time constraint to submit the newspaper ad proof and I didn’t want to take any risk. Passport correction takes 1500 rs.

Duration: It took me 1 month to get the appointment to PSK. Once I got it, they immediately processed it. So duration is variable here.

It is better to book an appointment a week after you submit your PAN application. This way, your proofs will be ready by the time you get your PSK appointment. Worst case, we can reschedule the PSK appointment 2 times.

Step 7: Changing name in any other places (like Credit cards, Mutual Fund houses, Gas, etc) can be done on the basis of any of the above ID’s.

Duration: Gas takes 2–3 working days, for others, it depends on the service provider.

  1. As per the information mentioned in the present query, makes it clear that you wish to change your surname in all other documents, but it can’t be changed in the 10th and 12th certificate if the period of passing the said exams has gone beyond the 5 years.
  2. You need to make an application one each concerned department during the reason as to why what to get it changed (marriage).
  3. Thereafter, you will get to done accordingly as per the rules of each individual department as per their own provisions.
  4. Then once you get the same as competed, you can inform the employer about the said change and the desire to adapt the same there also.

Dear Madam

You can change your surname by sending a formal letter for change of surname to your department.

With that you can provide the documents of marriage and IDs of your husband and your IDs in which your name of after marriage is mentioned.
