This is offence as per Indian constitution and you may file complaint to your nearest police station.

You can file a complaint to police station or write a letter to court or district collector informing the same.  The said practice is illegal and abolished long ago

You may write simple complaint/application to SHO of PS of your area. Do mention your name, address, mobile number etc. and the name and address of persons who committed the boycott against you and your family. Also mention in application who are the members of your family affected due to this activities of village men. Write  complete details of activities done by village. In prayer part, ask police protection and strong action those people. 

It is informed that by these acts of village men, your fundamental rights are affected and you may file writ in high court or supreme court if no action is taken by police in reasonable time.

Just write down normal complaint with details facts to incharge of station house. 

1.You shall have to collect evidence of their preventing you in entering the Mandir and other atrocities and then lodge a police complaint against them.

2. If police does not act, the file a Writ Petition before the high Court against police inaction praying for an order upon police to register FIR , investigate and act as per law based on your said complaint.

File a writ petition before high court if entry in temple and village area is not allowed and you are boycotted.

2. You should complaint about it to the state authority.
3. Then wait for few days, and file a Writ petition before the Hon’ble High Court seeking immediate prohibition against those you have been stopping you.

This type of social boycott is an illegal act, you may file a complaint with the collector and a copy to the local police station on this for relief and remedy.

For a format of complaint, there is no specific format in this connection, you may just like that express whatever has happened in your own language and get it signed by all the affected members of your family, present it to the district collector for further action and remedy to this
