• Same Person Affidavit

My father and mother opened a joint bank account with Axis at Navi Mumbai in 2003. At the time my mother was also known by a name given to her by my father-Mugdha Pradeep. This is the name registered in the bank records. 

In 2007, her PAN card was made. In the application we mentioned that Mugdha and Punam are the names of the same person (Punam is her real name-this is the name in her PAN record as well). 

In August this year, Axis bank freezed the joint account citing name mismatch. I contacted the Axis bank support, and they have advised me to get a duly notarized affidavit stating that Mugdha and Punam are the same person. 

Pl advice as to how to effect the same. Your inputs shall be highly appreciated.

Hello,

a same name affidavit has to be made and the same has to be printed on a 10 rs stamp paper and has to be duly notarized.

In this case you would simply need an affidavit stating mugdha and Poonam are 2 names of the same person and the affidavit should be notarized by a notary public after payment of a nominal fee. 

 

Once, you get this done the joint account citing name mismatch would be defreezed. You can attach a self attested copy of the PAN card if it shows both her names as well, just to be sure.

See on 100 Rs, Stamp a affidavit can be made stating that Poonam and Mugdha are one and same person the affidavit can be signed by two witnesses and further it has to be notarised by the Notary and same can be submitted to the bank along with copy of the PAN card.

1. As per law, she should execute an affidavit before the local 1st Class Judicial Magistrate affirming that she is known as both  Mugdha and Punam for which the Axis Bank is required to treat your name as Mugdha also. This is the requirement of all the PSU Banks.

 

2. However, if Axis Bank advised you to submit a notarised affidavit, get the above affidavit notarised and submit before the Axis Bank as advised by them to enable your mother to operate the said bank account.

hello

        go to a district court and contact an advocate. he will make an affidavit wherein it would be written that what is your mother's real name and what is her given name and that the names are of the same person.afterr that your mother would sign it. it would cost 200-250/- then deposit it with the bank. the bank would then proceed accordingly and defreeze the account.

You can prepare an affidavit stating that she is known by both the names and both the names belong to her. This affidavit may be notarised before a local notary public.
