Company cut the notice period without consideration..

Hi, I am chetan and I am working with a pvt ltd organisatin BPO since june 2013. Recently i asked my manager to grant me two months leave oe one month leave since i have few things to complete. I have almost 70 leaves in my account and my sr directly said no without saying a word. i didnt get any long leave in my entire work period as two week offs are enough in a week for me. When i needed they didnt think a bit and said to check for your options. I have no option but to resign. So i did that but I served a 30 days notice period. After 15 days of my notice period i was not able to go to office and after two days i had my two week offs. So i went afte rfour days and they denied my access in company and said that your notice was accepted for 15 days. Now i am relaxed that i dont have to serve for more days. So i asked for my full and final details from company. Then hr team said you need to pay for rest of 15 days since you dis not pay notice period. Now since i have no access i am not able to go inside the permises. I asked my supervisor for the reason. He said manager has accepted for 15 days notice. I sais i didnt say to accept my 15 days notice. Now they are asking 6000 without any reason. Also they added my surname in PF account while I dont have a single document which reflects my surname. I have single name inAll of my documents . Now they said it may take up to 8 months to change but not giving the surety. I asked for the reason. They had no asnwer. Now after serving 4 + years they are treating me like anything. Please suggest me what should i do. I am so upset. They ask me to come anytime.
