• Different names of same person in different documents

Hi my name is sanketh sindhe b s, I am presently preparing for UPSC CSE exam. My mother has two names 1.gayathri bai (before marriage)
2. Sadana Bai (after marriage)

I have used her 1st name in my every documents from 10th to degree certificates
But my mother's govt id's like Aadhar, pancard, voter id have her second name..

I'm worried that this might create me a problem during my selection, appointment or in future. Will UPSC or department of personal training (DoPT), verifies my mother name in my SSLC markscard with her govt id's

Meanwhile my father name is same everywhere in my original documents and his govt id's

Now I can't change the name of my mother in my original documents.. so is there any other options available for me . If yes what are they

I have heard about ONE AND THE SAME PERSON CERTIFICATE affidavit is accepted in these case.. what is the procedure to obtain it and what all documents are required so that UPSC or in near future Central government should accept it

You can change the name by Gazette notification and paper publication. You need to execute a simple affidavit along with it

Now you can make her Change in name affidavit and get state or central govt gazette of her second name, like her first name was gayathri bai (before marriage) and change to Sadana Bai (after marriage). And show this gazette to any job or employment purpose of yours to them.

Then only a notarised affidavit will suffice stating that both the names are of same person and the one is correct

Your mother can by notification in official gazette and 2 local newspapers change her name 

In alternative apply to school board And university to change your mother Name in your school and College records

- As per law, you cannot change or correct any names etc  in your academic certificate after maximum 2 years from the date of passing of the said qualifications. 

- Since, your fathers name is correct every where in all the certificates and documents , then you need not worry , as mothers name not come under verification for getting government job etc. 

- However, your mother can execute an Affidavit for the same  and publish the same in official gazette after mentioning that her both the names are same and is name of a single person.

Mother name never required for any verification but only father name. But avoid any confusion, obtain a gazette notification from collector office that gayathri bai and Sadana Bai both are one same person and is your mother.

There is no such Certificate of One and Same person.

That is an Affidavit not Certificate. 

So get an SAME PERSON AFFIDAVIT prepared and attested from notary

Hi

Since you have adapted your mother's maiden name in your academic certificates and whereas your mother has adapted her new name (post her marriage) in her ID Cards,  it is sufficient that your mother issues an affidavit under the One and Same person affidavit and the procedure is as follows:

1) You can download the template  available in many sites. However, we recommend that you download the template from  http://www.mca.gov.in/MinistryV2/formatofaffidavit.html

2) You can take the print out in Rs100-/- stamp paper.

3) your mother can fill in the blanks and sign thereafter before the Notary (they are available in court complexes).

The above procedure is generally sufficient. However you can also  opt to comply with the following steps:-

4) After the notarization of the affidavit is done, a newspaper advertisement about the one and same affidavit can be published in one in a local language newspaper and the other in a national daily. This should be easy as most newspapers have a section dedicated to such ads.

5) You can visit the Controller of Publication of any Government Gazette along with the attested affidavit, copies of valid ID proofs, copies of the published newspaper advertisements, marriage certificates and other important document and the one and same person affidavit will be published in government gazette which can be used by you all through your life. 


 6) Once you have the affidavit and also further published in gazette, no further steps are required from your side and you can use the gazette notification as and when required.

If your mother name mentioned in the SSLC certificate tallies with the name given in the application for UPSC examination, then you don't worry about other things.

The UPSC will not verify your mother's details, they will verify only your documents and your character and antecedents if you are selected for the job.

It may not be necessary for the desired affidavit.

However if you still are worried about it then you can prepare an affidavit duly notarised and get it published in a local newspaper.

It is not a certificate.

It is an affidavit which your other has to affirm on oath before a notary public.

You take your mother to a local notary public who will prepare the affidavit and notarize the same  and would guide you on further actions to be taken on this.

An affidavit must be made stating the facts ie your mother's previous name and why you stated her name so.

It must be notarized and submitted to the authorities.
