Hello,

You may make a same name affidavit and declare that both the name are of the same person.

You can not get your name changed in academic certificates now.

Also, if you want you can get your name changed in the ID cards and make it as per your academic records.

1)you would need court orders to add your surname in your academic records

2) the reason being school board/university would not make any changes after so many years without court orders

3) for renewal of passport mere aadhar card is sufficient

Yes, From now onwards, use your full name. Person identity dose not verify from education certificates but through identity cards like adhaar, PP, license etc.

No procedure just fill full name.

See now since time has passed the record in educational certificates cannot be rectified, it is better to change your Adhaar card and other documents as per the academic record that would be easy thing to do.

See you should apply based on your DOB certificate same date shall be considered for passport if DOB certificate is not there then in that case you can apply based on 10 marksheet .

1) you would face problems in renewal of passport as date of birth is different in passport and school leaving certificate .

2) for renewal write same date of birth as reflected in your old passport

3) in alternative get date of birth in passport changed by court orders as passport was made when you were minor

- Since, you have already compeleted the education upto Phd, hence after a long period , you cannot change your name in the academic records.

- The problem is , even court will not direct to school board, university etc , for a change in their records , simultanously.

- But, Passport, Adhar, Pan card etc can be changed similar to academic records.

- Yes, you can change your date of birth , at the time of renewal of passport, after submitting the 10th certificate and an affidavit.

- And , further on the same academic records and affidavit , you can change name in the address proof and identity proofs.

The date of birth can be changed in the passport.

For change of name in the academic records you have to make an affidavit and get it published in a newspaper and govt Gazette.

You cannot change your name in your academic records at this stage.

You can prepare an affidavit stating that you are known by both the names and both are one and the same, get it notarised  and then get the same published as public notice in a local leading newspaper, this will be sufficient for all your future requirements.

You better give the correct date of birth in the passport at least now.

If the date of birth differs from passport to that of the birth certificate then you can get it changed by giving an affidavit to this effect and attaching a photocopy of the original birth certificate.

You can get your passport reissued.
