See you need to first make a name change affidavit you then need to publish the name change in two newspapers and finally you can publish same in state/central gazettee notification.

Approach local notary in the court he shall prepare the affidavit and then you can accordingly complete procedure.

The right procedure is getting a notice published in a local News Paper, regarding your intention to change your name/or add surname to it, and subsequently get it published in Gazette of India.

Dear Sir,

Procedure for name change in educational certificates

Affidavit

Seek the help of a local notary to make an affidavit for name change. You will have to explain the reason why you want to change your name. For instance, in certain communities, a woman is given a new name post marriage and she may wish to change it legally, or someone may wish to change it because either it’s too long or difficult to pronounce.

Publish in newspaper

Next, you need to publish the change of name in at least two local newspaper. It’s recommended that you publish one in an English daily and one in a regional publication. This is to announce your new name publicly. Remember to keep copies of newspaper with you.

Gazette notification

Once it’s published, you should then get it notified in the official gazette of the state you reside in. This can be done by filling the deed changing name form from the gazette office. Don’t forget to take duplicate copies. Submit the copies to the office for publishing in the Government press for gazette notification.

Verification

Now comes the part where your documents will be verified by the Department of Commerce and Transport secretary . For this, you will need to send

– A letter to the secretary stating that you want to change your name and request him/her to allow you to do that.
– The deed changing name form.
– At least 2 copies of local newspaper, which published your name.
– Attested copy of the affidavit.
– A copy of your present identity (PAN card, voter ID card, passport, etc).

Approval

After receiving approval from the secretary, you will have to submit it to the local gazette office. You will also have to pay a fee for the office to publish it in the newspaper.

Having changed your name officially, now you can approach the educational institution and inform them about the new name. It would be helpful to visit the college administration office personally. Once you submit the documents ascertaining the change of name, then it’s only a matter of time before it gets incorporated in your educational certificate.

Name can be changed. Change of name should be published in the newspaper. An affidavit must be made and submitted and on the basis of that this change should be published in the government Gazette.

Broadly classified, the procedure for name change involves only three steps:

  1. Make an affidavit for change of name
  2. Place newspaper advertisement with details of change of name
  3. Submit papers to The Department of Publication

To change name by oneself, the first and foremost criteria to be met by a name change applicant is that he/she should be over 18 years age. The procedure is different for the name change of a minor.

An individual looking to change his/her name has to keep the below documents ready for the publication of change of name advertisement in the Gazette of India.

  1. An affidavit duly signed by the applicant and attested by Judicial Magistrate/Notary.
  2. Original newspaper in which name change ad is given.
  3. Prescribed proforma (should be computer typed and not handwritten) with signatures of applicant and two witnesses.
  4. A C.D. (compact disc) that contains the soft copy (typed content, not a scanned copy) of the application in MS Word format. In the place of signature of applicant, the applicant’s old name has to be given and witness details need not be included.
  5. A certificate in which the applicant declares that the contents included in both soft copy and hard copy are similar. The applicant should duly sign the certificate.
  6. Two passport size photographs, both self-attested by the applicant.
  7. A photocopy of a valid ID proof, self attested by the applicant.
  8. A request letter along with the requisite fee as per the Authority.

Apply For Change Of Name In Gazette

The documents listed above, printing charges along with a forwarding letter should be submitted to The Controller of Publications, Department of Publication, Civil Lines, Delhi-110054. The papers have to be submitted either personally or shall be sent by post/courier to the Department.

After that you should make application Before your board for change of name in certificates along with required documents and fees.

1, You have to change your name by firstly carrying out newspaper publication to be preceded by swearing an affidavit before a notary public.

2. Once the newspaper publication is done then you may apply for change of name on adhaar card but it cannot be changed on 10th and 12th marksheet as marksheets show the name which existed as on the date on which they were issued,

You can change your name as per your desire.

For that you may have to get a notarised affidavit and also get it published in a newspaper, besides, you may have to get it published in the government gazette which will be more authentic.

However, you cannot change your name in the existing educational certificates or the college certificate or records.
