• Addition of Surname

I'm a central govt. employee. I want to add my Family surname to my name as amongst other reasons, I guess it'll otherwise also lead to discontinuation in my family line. I would like to know the procedure for doing the same. Also, is it necessary to get it done in my own district court (permanent address) or can it be done anywhere? I'm ok with a paid service for a hassle free experience. Plz guide...Thanks.

It can be done anywhere.

You need to make an affidavit of name change assign surname to name. Then once affidavit is there newspaper publication of name change can be done in two newspapers

Since you are government employee in that condition you need to.compulosarily go for gazzeettee publication.

Send the name change application form along with prescribed fee and newspaper and affidavit copy to the diractorate/department of publication government of India,delhi.

90016/187-Estt. dated 12th March, 1987 from Department of Personnel and Training.

The Ministry of Home Affairs O.M.No. 67/274/48/Estts. dated 3rd November, 1948 as amended vide O.M. No. 87/52Estt. dated 24th March, 1952 lays down the procedure for change of name by Government employees.

The Govt. recently had the occasion to review these instructions in the light of experience in certain cases. It was found that the instructions are in complete in so far as they do not cover certain situations. It has, therefore, been decided to adopt the following procedure in suppression of all previous orders on the subject.

I. All cases of addition/deletion or change in name/surname:-

i. A Govt. employee wishing to adopt a new name or to effect any notifications in his/her existing name may do so, formally by a deed changing his/her name. The sample deed form is enclosed.

ii. The execution of the deed should be followed by publication of the change in a prominent local newspaper as well as in the Gazette of India at the Govt employee’s own expense.

II. Addition/Change in surname only, on account of marriage, remarriage of a female Govt. employees.

i. If the Govt. employee desires a change, she should give a formal intimation to her appointing authority of her marriage and request for a change in her surname.

ii. Particulars of the husband may be given for making necessary entries in the Service Book.

III. Deletion of surname or reversion to maiden name on divorce/separation or death of the husband of female Govt. employee, change may be permitted if a female Govt. employee gives:-

i. An intimations to the appointing authority regarding change in marital status; and

ii. A formal request for reversion to her maiden name.

For said work, you are suggested to get the affidavit attested by first class magistrate regarding name change and then get newspaper publication done. After that you may use your intended name change.

You can do it anywhere by publishing the intention to do so in one regional,one local and one national news papers , for national use English Paper and then submit the application with all the copies of above news paper physically to the following address if in Delhi if outside then by speed post to the following address:

You may have to follow the name change procedure. 

You prepare an affidavit,  get it notarized,  publish a public notice in a local newspaper and then get it published in the government Gazette publication. 

After this procedures you will be known by your new name.

For a central government employee : Use format of deed changing name/surname on the stamp paper instead of the normal name changing affidavit performa (link http://deptpub.nic.in/forms/form... ) and attest it from notary. You need to sign it from two people of your office as a witness and with there official address. It is good to choose gazetted officers of your department.

The procedure for name change involves only three steps:

  1. Make an affidavit for change of name
  2. Place newspaper advertisement with details of change of name
  3. Submit papers to The Department of Publication

 

 

If you are a Government employee, you can submit a deed for change of name in accordance with the rules stated in the Office Memorandum No. 19016/01/87 as issued by the Ministry of Home Affairs.

Furnish Documents Required For Name Change

An individual looking to change his/her name has to keep the below documents ready for the publication of change of name advertisement in the Gazette of India.

  1. An affidavit duly signed by the applicant and attested by Judicial Magistrate/Notary.
  2. Original newspaper in which name change ad is given.
  3. Prescribed proforma (should be computer typed and not handwritten) with signatures of applicant and two witnesses.
  4. A C.D. (compact disc) that contains the soft copy (typed content, not a scanned copy) of the application in MS Word format. In the place of signature of applicant, the applicant’s old name has to be given and witness details need not be included.
  5. A certificate in which the applicant declares that the contents included in both soft copy and hard copy are similar. The applicant should duly sign the certificate.
  6. Two passport size photographs, both self-attested by the applicant.
  7. A photocopy of a valid ID proof, self attested by the applicant.
  8. A request letter along with the requisite fee as per the Authority.

Apply For Change Of Name In Gazette

The documents listed above, printing charges along with a forwarding letter should be submitted to The Controller of Publications, Department of Publication, Civil Lines, Delhi-110054. The papers have to be submitted either personally or shall be sent by post/courier to the Department.
