1) gazette notification should be published of change of surname of child and also be published in 2 local newspapers
2) affidavit of name of child should be executed
Father can make an affidavit on name change of child and same can be published in 2 newspapers.
After that copy of affidavit and newspaper cutting can be sent to director of publication delhi for gazzettee publication.
This would require that you change the name of the child by first filing an affidavit and then publishing the same in local newspapers and then in the official government Gazette. Then the name change would become official.

One can change the name or surname of his child at any point of time before he attains majority.
There is no special procedure for this.
