you can choose either the mother’s surname or the father’s surname. This surname will be given to all subsequent children. 

You have to apply for change in surname and get advertisement in state government gazette.

1) gazette notification should be published of change of surname of child and also be published in 2 local newspapers 

2) affidavit of name of child should be executed

Father can make an affidavit on name change of child and same can be published in 2 newspapers.

At the time of admission of the child appropriate surname may be written in the admission form.  If already admitted file a representation for changing surname of the child accordingly.

If the mother and father of the child have no objection then simply obtain the Birth Certificate by the name you wants and that will be forever.
