• Affidavit for different name in degrees

Hi. I’m in the process of applying for a PR in Canada. I have a bachelors degree and a masters degree. My problem is that my name in my Indian Bachelors degree (university of Mumbai) has gone as 
(Last name) (First name) (father’s name) (mother’s name).
It diffrentiates from my actual name and the name in my masters degree which goes as (first name) (Second name) (surname).
What kind of legal document can I submit to prove that both documents belong to the same person. How can I get this legal document. Please help.

You haven't changed your name but the name was mispronounced in the previous degree. Therefore an affidavit should be made wherein you should declare your name as you are and submit it to the authorities. Also a corrected degree certificate should be applied for and they will give you another certificate which would have your correct name.

1. It is quite common that the spelling or name of a person remains different in different educational certificate.

2. This can be done by making an affidavit sworn in before the Magistrate and then make a paper publication.

3. Once this is done it is enough to remove the doubt , fi any.

You can execute an affidavit on Rs 100stamp paper that both are one and same person 

 

2) the affidavit should be duly notarised 

 

3) in alternative apply to university to change your name in its records

See if the writing style on degree of name is different then its good and legal you don't require any Affidavit for same as father's name and mother's name is written in the degree rest it is perfect.

Then also for your satisfaction you can make an affidavit on stamp paper and can get it notarised that name of bachlore degree so and so and name on master degree so and so are of same person and names are written as per University rules.

The only document you can submit is an affidavit sworn before an executive officer to vouch that name on the bachelors degree and masters degree is of one and same person. Nothing else can be done.

Seek the help of a local notary to make an affidavit for name change. 

Publish in newspaper. Next, you need to publish the change of name in at least two local newspaper.

1. You shall have to execute an affidavit affirming that your name has been wrongly mentioned in your certificate awarding you the master's degree as such and such in place of such and  such which is your actual name and also that the authority issung the master's degree certificate are required to correct the error and issue the certificate afresh with your correct name.

 

2. Thereafter you shall have to publish the gist of the affidavit in the Gazette and two Newspapers one being in Vernacular.

 

3. Finally you shall have to write to the authiority issuing your master's degree for issuing the said certificate afresh with your correct name.

 

4. Your problem will get solved now and you can apply for your passport in your corrected name.

You can prepare a notarised affidavit stating that both the names relate to you and ther is no difference in the names in whichever way it is called and both are one and the same.

You get the affidavit published as  a public notice in a local newspaper which will authenticate the issue.
