I have read all the requirements related to change of name - Affidavit, newspaper publication and gazette notification. 

My name is PRASHANT KUMAR and I want to change it to PRASHANT xyz.I will have to get the Affidavit counter signed by SDM/Tehsildar because my father's surname is not xyz. Do you know where will I find them? In collectariat or somewhere else


I am confused in the first step. Someone is saying you need to publish Affidavit on Rs 10/20 stamp paper but majority in patna is saying it will take Rs 100 stamp paper and Rs 15 ticket for welfare.I don't know law as per bihar stamp act. I am in two minds right now. I am thinking to print Affidavit on Rs 100 stamp paper just to be on the safer side. What should I do? please help.

Sir make an affidavit on 100 RS. Stamp get it notarised. No welfare ticket is required.

You will find the SDM in the office in collectoraite or if separate office is there inquire with local people.

Yes you may go ahead and get the samw on 100 rs to be on safer side, usually name change affidavits are sworn on Rs. 10/- stamp.

It is not necessary to get the name change affidavit to be counter signed by the SDM or a Tehsildar,

An attestation by a notary public would Sufi the requirement.

This is just a petty issue, for which you should not break your head so much, if you feel that Rs. 10- stamp paper may be better then your may go ahead, what is the confusion about it.

The motto is to get the affidavit for this purpose.

Tahsildar is found in Tahsil office which is situated in every tahsil.

Affidavit is always made on Rs. 100 stamp paper and it should be notarized.
