My mother has maintained her maiden name - Amita Anumeha in all her documents like PAN etc. However, in my passport , her name is mentioned as Amita Madhukar. I am now applying for an F1 VISA and I am scared I may face a problem with the same. She is a co-borrower in the Education Loan i have shown as proof of financial ability. Hence I am not sure of what to do. I do not have time to chnage the details in my passport. Is there some document I can prepare to show that she is the same person -Amita Madhukar and Amita anumeha ? Will I face a problem in getting my student VISA?

I am a lawyer by profession, and have followed the below given procedure for changing my own name. Let me use my experience to help you to change your own name.
Follow the below given steps for changing your own name.

You can choose any 2 local newspapers for publishing it. (No need for FrontPage coverage). Step 3 – Gazette Notification
Now for getting your name published in the Official Gazette of your state, for example: “The Delhi Gazette”, you need to do the following things:
1. Visit your local Government Press or the department that mobilizes Gazette notification.
2. Get 2 copies of “Deed Changing Surname Form”. (Fill this form carefully as per the instructions provided therein)
3. If possible get the phone number, or the mobile number of the person of that department who can help you for Gazette Notification.
Verification

1) affidavit should be on Rs 100 stamp paper
2) it should be duly notarised
3) you should get affidavit drafted by your lawyer
1 Make an affidavit done on this regard from the local first class Magistrate.

3. once both the things are done the anomaly with regard to name of your mother will be done way with and your problem would be resolved.
I advice you to do one more thing plus making that affidavit.
Give an advertisement also in the local newspapers about the same.

But, please be very accurate about the department where you need to go in the immigration department.
The US visa and the passport is given on yourname, wherein your mother's name has been found to be recorded as per the educational certificate.
It will be that only your documents will be verified and not your mother's own document.
There is no necessity for your mother's documents to be produced hence you cannot face any problem in that aspect.
However as an abundant caution you acane get a notarised affidavit stating that your mother's name is one and the same that which is shown in the educational certificate and aadhar card etc.

There is no format or a template for the affidavit.
You can visit any notary public's office and get the affidavit prepared at his office itself, get it and do the needful.
