• Change of name for 6 month old

We had our first child in March and my wife and I agreed on a name and got the birth certificate. We also got a passport for the baby. 
Subsequently we have decided to change the name due to some family and personal considerations. Both parents in agreement of the new name. Can you please advise on the right procedure.

You may get a notarized affidavit done to this effect, and then the same can be published in the newspaper and with these two things you may approach the passport office for issuance of a new passport with different name.

Execute an affidavit to change the name.

Notify the new name in some newspaper having wide circulation by means of a public notice.

on the basis of the notarised affidavit and the newspaper publication, approach the passport office to issue a fresh passport reflecting the new name.

1)he change of name of your baby cannot be made now in the birth register of the municipal/local authorities.

2) you can follow the procedure for change of name by executing affidavit , publishing in 2 local newspapers and publication in official gazette

3)the new name can be entered into in school records of your child

If once the birth has been registered it is not possible to get a new one and any ways if you will het the nsame changed you will be issued with a new certificate.

Since passport is already there, therefore it is advised that you get the name corrected everywhere.

1. The change in name would require you to follow a long drawn process to get his new name incorporated in the documents. If you have chosen a new name already then make a gazette publication and follow it up with a newspaper publication in a regional and national daily. The same procedure is to be followed even if you want to merely insert a middle name.

2. Birth certificate is issued only once irrespective of how many times the name is changed.

You can first prepare an affidavit for change of name of yor child, get it notarised and then file a petition to concerned government press for getting the name change published in government gazette and keep the copy with the other certificates to effect the name change.

1. Is there no way of getting a new birth certificate issued? The birth certificate is required for school admissions and if that is not changed it might cause confusion.

If the name has been registered with the birth registration authorities, then you have nothing to insist the authorities to change the name n the birth register

2. What if the only change we want is insertion of a middle name? We had kept only a first and last name for the child and now want to add a middle name.

Inserting any name in between and getting a new birth certificate in this regard may not be feasible.
