• Birth certificate without name - Amritsar

I am applying to be entered in the Swiss civil register to start my naturalization process. Mandatory requirement is the Birth certificate. I was born in Amritsar in 1964, and my birth certificate does not have my name written. Swiss authorities mentioned an affidavit can be obtained with School / other certificates. Both my parents have passed away , so I can not get the normal declaration / affidavit. 

Can some one guide me the process - I am getting my Birth certificate issued again (6 months requirement). But what to do next:
- affidavit contents / material
- certificate to be attached 
- has it to be done in Amritsar?

I am currently abroad, but have relatives who can help me in the process. Or do I need to come personally to India for this?

Affidavit has to be executed in Amritsar by close relative 

 

2) it should be on Rs 100 stamp paper or can be franked

Father can get affidavit made in Delhi as he is residing in Delhi

You can prepare an affidavit as well as a power of attorney deed,  execute it and get it attested by a notary public of the country of your residence,  send it to your relative back in India with instructions to complete the task as desired. 

You can approach an advocate either from this forum or outside for preparing the draft deeds on the terms of the chosen advocate.

If her mother's name is not found in the birth certificate,  she may not be allowed to add it at this belated stage. 

However she also can get an affidavit sworn to this effect and get it notarized which should suffice the requirement.

Affidavit can be made through lawyer and submitted to the concerned officer for adding the names. News Paper publication along with the same is necessary
