I am 22 years old. I do have a birth certificate but there's a problem in it. I was born in Raipur, Chhattisgarh in 1999. But the problem is that in 1999 Chattisgarh was Madhya Pradesh so it says Madhya Pradesh instead of Chhattisgarh. And also my passport says my place of birth as Chhattisgarh. I need to reissue my birth certificate for the US visa. Also my mother's name is not there on my birth certificate. What are the steps to correct these things in my birth certificate? How to reissue a new birth certificate?

on date of your birth Raipur was part of MP .hence birth certificate mentions MP as place of birth 

 

it cannot be changed to chattisgarh as no such state was in existence on said date 

 

approach authorities that issued your birth certificate to add your mother name in BC 

 

if they refuse you would need court orders to add mother name in BC

If the place of birth is correct then bifurcation of the state is not a problem.

You were born in the composite Madhya Pradesh which was later bifurcated by the government for the reasons made known in the government gazette, which is not your problem neither it would invalidate your birth certificate. 

The non inclusion of your mother's name in the birth certificate will not make the certificate invalid.

The inclusion of your mother's name in the birth certificate is not a mandatory requirement in the form or an application for visa to USA neither it is a requirement by applying for passport, hence there may not be a necessity to apply for reissue of passport for the said reasons.

A new birth certificate cannot be issued. It can be apostilled ie for use in a foreign country to confirm its authenticity.

An affidavit may be given by you that a new state was formed after 2000 and hence the problem. The government may attest to this fact. Local tehsildar/sdm may sign this document.

Birth certificate can be issued but for that you have to visit the municipal office of the city where you were born, pay a small amount of fee and then they will give you an application form to fill all your details required to make a birth certificate and they will provide you the receipt and date of issue. Now a days these procedure can be done online by visiting official website of municipal corporation.
