• I want birth certificate as per my choice of name

Dear Sir/Madam,

We live in Pimpri Chinchwad Municipal Corporation area in Pune city (Maharashtra). We have a daugher of 2.5 years old. She was born on 25th October 2016. I have applied for her birth certificate with the format- "Name Father-Name Mother-name". But the concerned authorities have given the certificate with format; "Name Father-Name My-Surname", which we don't want. Now, they are telling that we give certificate in this format only, even our software does not support such name formats! Actually, we don't want the surname in her name. 
We want to know, the laws which will support us to get expected format of birth certificate.

See you can get name change of your daughter vide affidavit newspaper publication and gazettee notification process though name in birth certificate cannot be changed now you have to use gazettee notification along her birth certificate for her further documents.

Dear sir,

Go to a municipal office nearer to you. Take the application form regarding the name change.

Go to local notary and make an affidavit. Specify the strong reason for change of name. This affidavit is used for changing first or last name or complete name. After the affidavit is prepared, submit it at municipal Office along with old birth certificate, Address proof, remaining proofs as well. Within 15 to 20 days, you will get your new birth certificate

The format of the Birth Certificate is designed as such.  You cannot change it according to your requirement.

However, in order to have the name format as desired by you, you need to get the same from obtaining orders from Competent Court of law. 

Other way round you can get the desired name published in the "Gazette" and intimating the General Public about the same by publishing in two local news papers one in local vernacular language and other in English language.

The Name shown in Gazette can be used by you, i.e., your chosen name.

You can change her name by gazette notification and delete her surname. Otherwise you need to approach the civil court to give directions to sub registrar if births & deaths

You can avoid surname in other docuemtns but in BC it is mandatory to avoid confusion of paternity. Whatever documents you may apply for her,mention her name without surname. NO bar neither any affidavit require.

Make an local Notary and make an affidavit regarding change of name without surname. Submit it in your respective District Court before a Magistrate or Notary Public.The Affidavit must mention the current name and the desired new name, and may mention in brief the reason for name change. The Affidavit must be made on a Non-Judicial Stamp Paper of minimal value.

The authorities are not willing to to rectify their mistake.

You may take up this matter with the district registrar by giving a representation  and request for the name to be printed in the manner you desire it to be, if he is also not listening then you ask him to give it in writing after which you can approach court of law for relief and remedy.
