Normally the head of the department is the authority who Grant's noc. In your case the BSA should be the competent authority.
Write an application to the BSA and DIOS both.

Visa NOC for Uttar Pradesh Government employee in Basic Education Department
My wife is government employee in UP Government under Basic Education Department. We are going for Malaysia and singapore for family trip during summer holidays, Singapore Government needs NoC while Malaysia Govt dont need. How we should proceed for her NoC from the department. Who can Authorise BSA or Secretary

as per existing instructions, when a government servant applies for leave for going abroad on a private visit, separately prior permission of the competent authority for such visit is also required.

Your HOD can issue NOC in the said matter. Else you need to file writ petition in HC
Talk to her boss who would guide her regarding the competent authority to grant NOC.
Generally the Secretary or Under Secretary does this work.
The appointing authority of your wife is competent to issue her this NOC.
Write in this matter to the BSA.
See your wife can write a letter seeking NOC from the BSA and letter can be CC to Secretary. The head of department issues such NOC.

Let she first apply for NOC from her department on this, the administrative head may, on receipt of the application, issue instructions to her to comply with the requirements in this regard, if they dont and hey refuse then let she approach the higher authority and seek the same where she will get some or the other relief, if nothing is made available then you may approach court seeking direction against her department for issuing the said NOC.
