One and the same person Certificate
My mother's name is without surname on my documents, but it’s there on her’s. The Italian Consulate(Mumbai) asked for “One and the same person declaration” for her from Tehsildar & they said an affidavit is inadmissible for visa. What’s the process to get it in Maharashtra?

This is the document the Italian consulate has sent. Declaration from the competent Registrar of Births & Deaths, Grama Panchayat or Tehsildar, stating that : - XXX (name of father on the birth certificate of the son in Italy, and on the father’s marriage certificate), YYY (name of the father on his son’s passport), and ZZZ (spouse’s name on the mother’s passport) are one and the same person. - MMM (name of mother on the birth certificate of the son in Italy), NNN (name of the mother on her passport), are one and the same person. The above mentioned declaration has to be legalised by the Under Secretary of the competent Home Department of the State, apostilled by the Ministry of External Affairs, New Delhi. Kindly note that an affidavit for discrepancy in name is not legal tender for Italy.

You should approach local tehsildar in this regard for assistance
2) enclose format given by Italian consulate
3) an affidavit on Rs 100 stamp paper that both are one and same person
Visit the MRO office with the affidavit and ID proof and along an application the MRO/tehsildar shall make an certificate of one and same person. your mother has to visit along.
Further you can also seek an court order for declaration of the mothers name.

You can approach the office of the local Tehsildar, give a written request about it, if needed, with an affidavit and the format in which this certificate to be issued, understanding the genuineness, the tehsildar may be kind enough to oblige.
There is no legal provision for this.
You can approach the concerned officer, if he refuses to furnish the desired certificate you may approach court seeking direction to the concerned authority for doing the needful.
