My mother's name is without surname on my documents, but it’s there on her’s. The Italian Consulate(Mumbai) asked for “One and the same person declaration” for her from Tehsildar & they said an affidavit is inadmissible for visa. What’s the process to get it in Maharashtra?

This is the document the Italian consulate has sent. Declaration from the competent Registrar of Births & Deaths, Grama Panchayat or Tehsildar, stating that : - XXX (name of father on the birth certificate of the son in Italy, and on the father’s marriage certificate), YYY (name of the father on his son’s passport), and ZZZ (spouse’s name on the mother’s passport) are one and the same person. - MMM (name of mother on the birth certificate of the son in Italy), NNN (name of the mother on her passport), are one and the same person. The above mentioned declaration has to be legalised by the Under Secretary of the competent Home Department of the State, apostilled by the Ministry of External Affairs, New Delhi. Kindly note that an affidavit for discrepancy in name is not legal tender for Italy.
