e) An extract of Government Service Record (book) mentioning caste/community category of applicant’s father or relative.

3) Additional Documents for Caste Certificate
Along with above mentioned common documents required for caste certificate, you may need some additional documents as per scenario. In this section, we will try to cover all of those scenarios and lists of document required for each scenario.
Case i) Father’s caste certificate not available: Then you need to have your relative’s caste certificate along with the genealogical affidavit. This affidavit explains family lineage and history.
Case ii) Migration from Other State or District: If you have migrated from other state or district then you need to provide father’s caste proof issued by a state or district from which you have migrated.
