The Govt. recently had the occasion to review these instructions in the light of experience in certain cases. It was found that the instructions are in complete in so far as they do not cover certain situations. It has, therefore, been decided to adopt the following procedure in suppression of all previous orders on the subject.
I. All cases of addition/deletion or change in name/surname:-
i. A Govt. employee wishing to adopt a new name or to effect any notifications in his/her existing name may do so, formally by a deed changing his/her name. The sample deed form is enclosed.
ii. The execution of the deed should be followed by publication of the change in a prominent local newspaper as well as in the Gazette of India at the Govt employee’s own expense.
II. Addition/Change in surname only, on account of marriage, remarriage of a female Govt. employees.
i. If the Govt. employee desires a change, she should give a formal intimation to her appointing authority of her marriage and request for a change in her surname.
ii. Particulars of the husband may be given for making necessary entries in the Service Book.
III. Deletion of surname or reversion to maiden name on divorce/separation or death of the husband of female Govt. employee, change may be permitted if a female Govt. employee gives:-
i. An intimations to the appointing authority regarding change in marital status; and
ii. A formal request for reversion to her maiden name.
NOTE: - There is no prescribed form for items II and III.

Yes, NRI change of name is possible by taking embassy permission on change name process.

See you can come to India and you can make an affidavit of name change , then you have to do paper publication that is in 2 newspaper name change advertisement after same you can do a gazeettee notification in gazeettee of state or India.

However the procedures for change of name is very simple, you may have to prepare an affidavit stating the change of name from previous to the currently proposed one, give an advertisement in a local newspaper and if necessary get it published in the government gazette publication which will suffice your requirement.

You may see what best can be done after going through various opinions by various lawyers of this forum.
1. You shall have to execute an affidavit before a 1st Class Judicial Magistrate of your area in India affirming that you have changed your name from Mr. X to Mr. Y which is to be recorded by all authorities including passport authorities.
2. Thereafter you can send the copy of the said affidavit to all the authorities for amending your name accordingly.

- Generally, the name change post 35 age is very difficult to obtain however if the cause is serious and required for some genuine purpose, you can obtain the same
- You shall have to file the petition in Civil court with requisite documents along with stating the cause of requirement. If the Civil Judge is satisfied with same he/she shall order for the change in name. You are not required to visit the court and lawyers on your behalf can contest the matter.
