Dear Sir/madam,

My name is Malay Rajendrakumar Panchal. My problem is regarding me and mine mother relationship proof on paper.. The whole problem I have to faced because of name change and differ. My mother's original name before first marriage was Manisha Prabhudas Panchal for which mentioned on school leaving certificate. After first marriage her name change to Smita(Smita Rajendrakumar Panchal) for which we don't have any name documents except on divorce paper. My mother had had divorced after long divorced court case of 10 years and taken custody of children with her only. At that time my age is 15 year. So after divorced my mother name changed to Smita prabhudas panchal, for which we have election card of my mother written the same. After that my mother got married second time and now her name was Manisha Devendra Ajmeri, Manisha name taken from her school leaving on marriage certificate, my mother has now all documents like pancakes, adharcard, passport, on the name of Manisha Devendra Ajmeri only.
Now I am living with my mother only with my step father, we are family. Now problem is on my birth certificate my mother name written 'Smita' only. Now I don't have any proof which proof Manisha Devendra Ajmeri is my mother, so I facing problems at the time of making passport, for my address proof as a step father address, even i am not able to made nomination of my mother for insurance policies as i am not able to provide relationship proof documents. I have not at a any relationship with my actual father whom i meet only 3 to 4 times only in my entire life.. I always live and grow with my mother only but now i dont have any legal proof documents to proof that Manisha devendra Ajmeri is mother of my self Malay Rajendrakumar Panchal. My age is 27 year now. Kindly help me out for this and guide us for this issue of my life

1. The anomaly in names or spelling in it is not a great problem and it can be easily dealt with.

2. Fir this your mother needs to make an affidavit before the first class judicial magistrate and a newspaper publication.

3. Once that is done the confusion over the name of your mother can be aptly dealt with.

You must apply to registrar of birth for correction of mother name in your Birth certificate

It should be supported by affidavit of your mother

Procedure would be a bit long but effective, ask your mom to give an affidavit stating all the details in it then publish that in the local newspaper and then finally go to each department and get your detail changed to as per the affidavit.

Now the process is online also for school and college certificates.

Dear Client,

Surprising n foolish, even ur mother is under identity cheating.

In all id`s, father`s name includes not mother , y going with mother name, use old ids n get passport.

Better ask your father to adopt u from your mother by this u will become legitimate child of your new father and will acquire all inheritance right as well which u don`t have at present.

Than on basis of adoption deed, change name of father in all ids.

You don't have to be so much concerned about your mother's name at this stage.

The name appearing in birth certificate may be mentioned in the passport.

For insurance purposes you get a notarized affidavit stating that the names mentioned in birth certificate and that of other documents pertains to her.

Keeping the copy of this notarized affidavit with the other documents will save your time explaining the everywhere
