How to get an affidavit from first class judicial first class magistrate?

Hi I have been asked to produce an affidavit(along with other documents) attested by Judicial first class magistrate by the District Form Officer(Gazette notification printing office) for adding a document detail in gazette notification. I had changed my name officially through gazette notification in 2009. In that notification I had mentioned all the documents such as passport, 10th certificate, election ID card etc. But I didn't mentioned the "Physical share document" in that gazette notification, which was bought back in 2002. Back then they won't allow share documents to be included in gazette notification and I forget to include that. But when I recently tried to change the physical share certificate(name is wrong in this doc) to demat form, the name conflict arises and they(share registrar) needed a gazette notification(along with other docs). So I talked to the district form office(govt press), they told me that in order to include the share certificate details in new gazette notification, I need to get an affidavit from Judicial first class magistrate, ad in 2 newspaper etc. So it would be a great help if some kind person would help me in 1) how to get the affidavit attested from magistrate 2) format of the affidavit for adding a document detail in gazette 3) the non-judicial stamp paper value, stamp value.
